Establishing an Account and Placing an Order
STEP A:
All Orders
If you are purchasing our Microarrays or our DNA-to-Data Service, you will need to send us three documents to get started:
- Set up your account by completing the Cancer Center Core Authorization Form >. The form should list all authorized users for a specific account or cost center and be signed by the authorized account administrator.
- Tell us background information on your project to be sure we fulfill your order correctly by completing a Project Information Form.
- Provide us with a Written Order (P.O., letter, or email) that specifies the exact product or service you would like, quantity, price, and total purchase. In this Order, please provide us with your FedEx or UPS account number to use for all shipping, insurance, and import duties, if applicable.
Fax these forms to:
Greg Hamilton (415) 476-8218
AND
Sandy Ng (415) 353-9728
Your account will be established the next business day. We will then email you your username and password to use on our CoreAdmin Site, where you must officially enter your order before we can begin work.
If you are only purchasing our Microarrays, this is all you need to do. We will contact you shortly to confirm your shipment. If you are ordering our DNA-to-Data service, please continue to Step B.
STEP B:
DNA-To-Data Service
Please provide us with three additional items:
- Fax a copy of your IRB (Institution Review Board) Approval to use Human samples if your DNA samples are from human clinical material (fresh, frozen, or paraffin embedded). This form is not necessary if the DNA is from cell lines.
- Fax the 260/280 and 260/230 spectrophotometer readings and concentrations for each sample you submit. You can use our Spectrophotometer Form or one of your own design. Samples with 260/280 ratios less than 1.8 and 260/230 ratios less than 2.0 can lead to poor hybridization results and should be discussed with the Core before sending samples.
- A minimum of 1 ug of sample DNA (preferably 2 ug) suspended in 10 uL of biology grade water is preferred. We recommend sending samples on ice.
Fax these forms to:
Greg Hamilton (415) 476-8218
We will confirm the receipt of your samples and provide an estimated shipping date by email.
